Facilitating Successful Re-entry

The Behavioral Health team at the Pueblo County Detention Facility is dedicated to overcoming the challenges of community re-entry for formerly incarcerated individuals. Leaders recognized the need for specialized behavioral health resources tailored to re-entry and ongoing substance use disorder recovery, beyond what community health centers provide. To enhance support, they are partnering with law enforcement to implement diversion programs aimed at keeping individuals out of the system. The pandemic complicated outreach efforts, leaving case managers with only insecure phone calls.

To facilitate consistent communication and improve the re-entry process, Pueblo County Detention Facility has partnered with CHESS Health to deploy the Connections app.
